ชมรมศัลยกรรมจักษุตกแต่งและเสริมสร้าง ได้จัด Symposium ในการประชุมราชวิทยาลัยครั้งที่ 31 ที่ผ่านมา ในหัวข้อเรื่อง Thyroid Eye Disease "From Past to Present" : Changing to Better Management โดยมุ่งเน้นเสนอการพัฒนาและการเปลี่ยนแปลงในการดูแลคนไข้กลุ่ม Thyroid Eye Disease ทั้งการวินิจฉัย การรักษาโดยให้ยาหรือการผ่าตัด

Classification
    การแบ่ง classification เพื่อการติดตามการรักษาและเพื่อประโยชน์ในการตัดสินใจแนวทางการรักษาว่าสามารถใช้ยาได้ก่อนหรือต้องผ่าตัด  ซึ่งการแบ่ง classification ที่ดีนั้นต้องสามารถบอกได้ถึงระดับความรุนแรงของโรค (severity & activity) แนวทางการรักษา (guide of treatment) และการพยากรณ์โรค (progression)

    วิวัฒนาการในอดีตแบ่งเป็น class 1 และ 2 ต่อมาเริ่มนิยมใช้ NOSPECS แต่ข้อจำกัดคือไม่สามารถบอกได้ถึง activity ทำให้ไม่ช่วยในการตัดสินใจในการรักษาและการติดตามการรักษาได้ ต่อมาจึงมีผู้คิดค้น EUGOGO และ VISA score ขึ้นมา ที่สามารถบ่งบอกได้ถึง severity และ activityของโรคได้ดีขึ้น

EUGOGO ทำการแบ่ง ดังนี้

Severity : 1.Sight-threatening  : Optic neuropathy 
                                               Corneal breakdown
               2.Moderate to severe  : Any one or more of following
                                                  Lid retraction ≥ 2 mm.
                                                  Exophthalmos ≥ 3 mm. above normal
                                                  Diplopia
                                                  Moderate or severe soft tissue involvement
               3.Mild : Signs less than moderate to severe disease
                           Corneal exposure response to lubricants

Activity : Clinical Activity Score (CAS)  :  disease active  ≥ 3/7
                     Orbital pain at rest                 0-1
                     Orbital pain on gaze               0-1
                     Lid redness                            0-1
                     Conjunctival redness               0-1
                     Lid swelling                            0-1
                     Conjunctival edema                0-1
                     Caruncle / plica inflammation  0-1
            
            : Follow up  : as above add 
                     Increase ≥ 2 mm. in proptosis                                  0-1
                     Decrease ocular motility in any direction≥ 8 degree    0-1
                     Decrease VA 1 Snellen line                                       0-1
  
ข้อจำกัดของ EUGOGO คือ คะแนนไม่สามารถบอกได้ถึงserious complication เช่น optic neuropathy และแต่ละลักษณะที่ตรวจพบได้คะแนนเท่ากัน เช่น optic neuropathy คะแนนเท่ากับ conjunctival redness

VISA Classification แบ่งอาการตามคลีนิค ดังนี้

V : Vision     (VA, color, RAPD, optic nerve)
      Goal : R/O optic neuropathy = Sign of compressive optic neuropathy : crowded orbital apex,         
                increase SOV diameter, ON stretching, anterior displacement of lacrimal gland
      Treatment : high dose steroid, orbital decompress, adjunctive RT

I : Inflammation   
          Retrobulbar ache        0-2
          Lid redness                0-1
          Conj. Redness            0-1
          Lid edema                  0-2
          Chemosis                   0-2
          Caruncle edema         0-1
          Diurnal variation         0-1
     Treatment : Score  ≤ 4/10 : conservative
                       Score  ≥ 5/10 : oral/iv steroid, RT, immunosuppressive agents

S : Strabismus
     Treatment : Inflammation       - Patching one eye, Prism
                       Non inflammation – Prism, Muscle surgery

A: Appearance/Exposure
      Proptosis, lid retraction, lagophthalmos, corneal erosion or ulcer
      Treatment : Supportive, aggressive lubricants, medication, radiotherapy, surgery (tarsorrhaphy, orbital decompression, muscle surgery)

Management of Thyroid Eye Disease

1. Non surgical Treatment
      Lubricant, moisture chamber
      Steroid : oral, local, subconjunctiva, retrobulbar, intravenous
      Orbital radiation 10-20 Gy (only adjuvant) 
      B cell targeting monoclonal Ab : Rituximab
      Dietary supplement : Selenium

2. Surgical Treatment : Orbital decompression 
      Indication : optic neuropathy, exposure keratopathy, cosmetic

      Pre-op : Stable TFTs, stable clinical measurement, stable photographs review,  surgical risk     
                   discussion (diplopia, blindness, infraorbital hypesthesia)

                 : Recent CT orbit, size of EOM, evaluate bony orbit, sinus disease

      Surgical planning : Estimate amount of proptosis reduction
                                  2 mm/wall , fat resection 0.7 mm/1cc.

      Complication : double vision, globe displacement, chronic sinusitis, CSF leakage, meningitis, optic  
                            neuropathy, orbital cellulitis, orbital Hemorrhage, under/over correction
