The iCare IC200 tonometer produces IOP measurements similar to those from the GAT
In a cross-sectional study, IOP measurements
taken with the iCare IC200 were compared with measurements taken with the Goldman
applanation tonometer (GAT). One hundred sixty-eight adult
eyes were included. First, 2 IOP measurements were taken with
the GAT; afterwards, the iCare IC200 measured IOP in the
upgaze, downgaze, medial gaze, and lateral gaze positions and
then at 2 mm from the limbus in the superior, inferior, nasal, and
temporal cornea. There were no significant differences in
the GAT and iCare mean IOP
measured at the central cornea (the only location where the GAT was used), but IOP measurements taken with the IC200 in different angles of the eye and different locations on the cornea were statistically significantly lower than those taken in the
primary position and at the central cornea. However, these differences were relatively small and thus may be clinically insignificant.

Intravitreal Ziv-Aflibercept Was Noninferior to Bevacizumab for Treating Macular Edema
A double-blind, placebo-controlled, randomized trial compared the efficacy of intravitreal ziv-aflibercept (IVZ) and bevacizumab (IVB) injections for the treatment of macular edema secondary to central retinal vein occlusion. Twenty-four patients received 3 monthly injections of either IVZ 1.25 mg/0.05 mL or IVB 1.25 mg/0.05 mL. Mean BCVA and central subfield thickness (CST) in both treatment groups significantly improved from baseline as early as month 1; these improvements remained at month 6. Visual and CST improvements seen in the IVZ group were better than those seen in the IVB group at 6 months, but this difference was not significant. No ocular or systemic adverse events were observed.

In Thai patients seeking refractive surgery, the prevalence of keratoconus is low
An observational study evaluated the prevalence and
characteristics of keratoconus in 1923 patients who
sought refractive surgery at a single hospital in Thailand over a
3-year period. The keratoconus and keratoconus suspect prevalence rates
were 1.66% and 0.68%, respectively; 74% of those with either condition were male. Seventy
percent of patients presented with either Stage 1 or Stage 2
keratoconus. The most prevalent ocular signs were the Fleischer ring,
Vogt striae, and Munson sign. Mean uncorrected visual acuity was
0.88 + 0.64 logMAR, and mean BCVA was 0.40 + 0.49 logMAR.
Fifty-seven percent of cones were oval and 33% were astigmatic.

Damage to the peripheral
ocular surface may be a hallmark of severe dry
eye
A cross-sectional study conducted in 92 patients with dry eye disease (DED) used spectral-domain OCT to identify the characteristics of corneal epithelial thickness and assess whether epithelial thickness correlated with DED clinical severity. The results showed no significant differences in overall epithelial thickness between patients with severe DED and patients with non- severe DED; however, the peripheral corneal epithelial thickness variance was significantly higher in the severe DED group. This suggests that there is greater damage to the peripheral ocular surface in severe DED, and thus this clinical finding could be used as marker of DED severity.

Glaucoma is a rare complication of FEVR in children, but it can be managed
Childhood glaucoma secondary to familial exudative vitreoretinopathy (FEVR) is a rare complication caused by later stages of the disease. In a retrospective, cross-sectional study, final visual acuity and intraocular pressure related to 2 types of childhood glaucoma secondary to FEVR were reviewed from the records of 144 children. Eleven eyes eventually developed childhood glaucoma; the mean time from FEVR presentation to glaucoma diagnosis was 42.2 months. Three of 9 eyes with neovascular angle closure required glaucoma surgery, while 2 eyes with non- neovascular angle closure were treated with trabeculectomy. Ten of the 11 eyes had an IOP of <21 mm Hg at the final follow-up visit.

Modified G-ROP screening criteria are validated for use in Thailand
A retrospective chart review was used to validate postnatal growth and retinopathy of prematurity (G-ROP) screening criteria for a Thai cohort. Six hundred eighty-four infants who met 21 of the following criteria for G- ROP were included in the study: birth weight <1051 g; gestational age <28 weeks'; weight gain (WG) <120 g during postnatal days 10-19; WG <180 g during postnatal days 20-29; WG <170 g during days 30-39; and hydrocephalus. The ROP prevalence rate was 26.6%, with 26 infants requiring treatment. The investigators
adjusted the G-ROP criteria to account
for the initial eye examination occurring at 4 weeks' postnatal
date, with the last 2 criteria replaced by the occurrence of
grade 3 or 4 intraventricular
hemorrhage. The sensitivity of these modified G-ROP criteria for identifying
type 1, type 2, or severe ROP cases was 100% with 42.5% specificity, excluding 271
cases of unnecessary screening.

During intravitreal injections, having patients
not talk reduces bacterial dispersion
An experimental crossover study investigated whether fenestrated surgical drapes (covering the nose and mouth but with an opening over the periorbital area), either with or without patient surgical face masks, increase periorbital bacterial dispersion. Sixteen healthy volunteers each performed 14 scenarios involving different mask and drape conditions, plus either speaking or remaining silent. Speaking situations showed significantly more bacterial dispersion, in terms of colony-forming units (CFUs), than any other situation. Wearing masks vs not wearing masks and using vs not using drapes did not significantly affect the number of CFUs. The authors conclude that the most effective method of reducing bacterial dispersion and the risk of post-injection endophthalmitis is to have the patient refrain from speaking.

A combined nimodipine-alteplase infusion improves vision in patients with CRAO
In this prospective study, selective
intra-arterial thrombolytic therapy (IAT), consisting of
nimodipine plus alteplase, was found to significantly improve BCVA at
1 month post-treatment in 9 patients with acute
non-arteritic central retinal artery occlusion (CRAO). Patients were
included if the time of CRAO onset to the predicted time of treatment
was <24 hours. Minor adverse events during administration
of the nimodipine infusion, including chemosis and headache,
resolved after discontinuation of treatment. Delayed hemorrhage was seen
in 3 patients following alteplase infusion.

Resolving ocular gnathostomiasis relies on timely parasite
removal
This report features a case of ocular gnathostomiasis presenting with acute anterior uveitis and uveitis glaucoma in a 56-year-old man referred to a tertiary eye center. A nematode was found in the right anterior chamber and was later identified on pathologic examination as Gnathostoma spinigerum. Surgical removal of the nematode was successfully performed. The authors suggest that early detection of parasites and their timely surgical removal is important for successful management ocular gnathostomiasis.
