Extended ILM peeling yields higher closure rate for full thickness
macular hole
Researchers compared 1- and 2-disc diameter conventional internal limiting
membrane (ILM) peeling for large full thickness macular holes. The extended
procedure led to a higher closure rate, particularly among patients with
chronic full thickness macular holes for more than 6 months. However, visual
outcomes were comparable in both groups. Eye, November 2020

IOP readings of edematous eyes appear to be lower with GAT
than other tonometers
Researchers assessed IOP of 60 patients with corneal edema after
phacoemulsification using 4 different tonometers. Pressure reading with
Goldmann applanation tonometry (GAT) tended to be lower than those obtained by
the other tonometers, particularly the Tono-Pen. Caution should be used when
interpreting GAT IOP readings of edematous corneas, the authors conclude. Journal

Early surgery and amblyopia treatment should be considered
for childhood glaucoma
This retrospective study identified prognostic factors for poor visual outcomes
in 45 glaucomatous eyes of children 4 years or younger. Approximately 64% of
eyes had poor visual acuity, often due to deprivation amblyopia. Prognostic
factors for poor vision included secondary glaucoma, a diagnosis at 3 months or
younger, and an interval of 3 months or longer before surgery. Journal of

Endogenous endophthalmitis yields poor visual outcomes
even after treatment
This retrospective study describes 29 eyes with endogenous endophthalmitis. The
most common causative organism was Streptococcus spp. Positive vitreous
cultures were significantly tied to unfavorable visual outcomes. Even with
early treatment, the rate of evisceration and enucleation remained high. Journal

Retained IOFB linked to poor outcomes in post-traumatic
endophthalmitis
Researchers report prognostic factors for poor visual outcomes in 66 patients
with post-traumatic endophthalmitis after open globe injury. Analysis revealed
that a retained intraocular foreign body was significantly linked to poor
visual outcomes. Most cases were caused by gram-positive organisms,
particularly Bacillus cereus. International Journal of Ophthalmology,

Baseline vision, steroid treatment affected visual
outcomes in NMOSD-ON
This retrospective study identified prognostic factors for visual outcomes
after the first episode of neuroretinitis optica spectrum disorder-related
optic neuritis (NMOSD-ON). Assessment of 63 eyes revealed that a baseline BCVA
of counting fingers or better and intravenous methylprednisolone treatment
within 21 days of symptom onset were factors for good visual outcomes. Clinical

Recurrent Vogt-Koyanagi-Harada disease linked to altered
choroidal volume
The authors examined 3D choroidal volume changes in patients with recurrent
Vogt-Konayagi-Harada disease. Using enhanced depth imaging OCT, they found
alterations choroidal volume during the recurrent stage of disease, even in an
absence of anterior segment inflammation. The findings suggest central
choroidal volume may serve as a biomarker for detecting choroidal morphological
change in this patient population. Current Eye Research, November 2020

Patients with MOG-IgG optic neuritis tend to have better
visual outcomes
A new study describes the clinical characteristics and long-term visual
outcomes of optic neuritis in Thailand. Researchers assessed 28 eyes with
MOG-IgG optic neuritis and 59 with AQP4-IgG optic neuritis during an 11-year
period. Patients with MOG-IgG disease tend to present with bilaterality and
optic disc edema and have better visual outcomes compared with patients with AQP4-IgG
disease. Clinical Ophthalmology, November 2020

Genetic polymorphisms tied to severe ocular complications
in acetaminophen-related SJS/TEN
The authors investigated the role of genetic polymorphism in 20 Thai patients
with acetaminophen-related Steven-Johnson (SJS)/ toxic epidermal necrolysis
(TEN). Within the cohort, severe ocular complications was strongly associated
with human leucocyte antigen (HLA)-B*44:03 and moderately association with
HLA-A*33:03 and HLA-C*07:01. British Journal of Ophthalmology,

Vitrectomy provides favorable anatomical success in AIDS
patients with retinal detachment
This analysis evaluates outcomes in 45 patients (52 eyes) with AIDS undergoing
vitrectomy for cytomegalovirus retinitis-related retinal detachment. Over a
mean follow-up of 42 months, primary anatomical success was achieved in 86% of
eyes and 65% had a VA of 20/200 or better. The use of highly active
antiretroviral therapy prior to surgery and early vitrectomy increased the
probability of achieving both anatomical and VA success. Medicine

PFCL use can yield satisfactory retinal reattachment
A new study describes the outcomes of retinal detachment repair by vitrectomy
and perfluorocarbon liquid (PFCL) tamponade. Among 122 study eyes, short- to
medium-term PFCL tamponade achieved a high satisfaction rate. Post-operative
hypotony was a predictor for unfavorable visual and anatomical outcomes. BMC

Characterization of HIV-associated optic neuritis
These findings suggest CD4 cell counts and viral load may help identify the
type of acute optic neuritis (ON) in patients infected with HIV. Among 10 study
patients, those with infectious ON had low initial CD4 counts and high viral
loads while patients with noninfectious ON had increasing CD4 cell counts and
undetectable HIV viral load after antiretroviral therapy. International

Some patients may benefit from using an eye drop guide for
self-administering medication
Researchers describe the usefulness of an eye drop guide device for
self-administering glaucoma medication. After 4 to 6 months, approximately 19%
of 57 patients used the guide everyday while 46% never used it. The findings
suggest an eye drop guide is most effective for patients using only 1
medication administered in a standing or sitting position. Clinical

Glycerin-preserved corneoscleral patch grafts effective for GDD implantation
This study investigates outcomes for glycerin-preserved human-donor
corneoscleral tissue as the patch graft for glaucoma drainage device (GDD)
implantation. Among 102 glaucoma patients, the 5-year survival rate of the
corneoscleral graft was 96%. The patch graft-related complications were

CMET can lead to cornea-like epithelium in successful cases
A 13-patient study suggests successful autologous cultivated oral mucosal
epithelial transplantation (COMET) can lead to a predominant cornea-like
epithelium at long-term follow-up. Failed cases had a mainly conjunctival
phenotype. However, all eyes had evidence of oral mucosal epithelial cells. Cornea,

Advanced stage presentation hinders effective management of retinoblastoma
This retrospective study assessed whether new therapeutic approaches have
improved clinical outcomes in patients with retinoblastoma in Thailand. Medical
records of patients treated at a single hospital revealed that survival rates
and globe-saving rates appear to have improved but advanced stage presentation
remained the major obstacle. Pediatric International, October 2020
