Visual Impairment in Older Adults Is Often Associated with
Manageable. Eye Diseases
Investigators reviewed medical charts of 1452 patients 260 years of age at a university eye clinic in Thailand to determine the prevalence and causes of visual impairment. When visual impairment was defined using presenting visual acuity, the rates of moderate-to-severe visual impairment and blindness were 69.6 and 6.2 per 1000 persons, respectively; the same rates were 11.0 and 0.7 per 1000 persons when using BCVA. Many eye conditions associated with visual impairment in this study were manageable diseases (e.g. cataract, glaucoma), suggesting that promoting eye health awareness and disease screening may help to prevent vision loss. Retinal Nerve Fiber Layer Thickness Could Help Predict
NAION Visual Outcomes
This retrospective study evaluated the effectiveness of using peripapillary retinal nerve fiber layer thickness (pRNFLT) to predict favorable visual outcomes in 60 patients with acute nonarteritic anterior ischemic optic neuropathy (NAION) at the 6-month follow-up visit. Favorable visual outcomes were defined as visual acuity (VA) z20/25 and a visual field index (VFI) 290%. Thinner temporal-quadrant pRNFLT was associated with favorable VA, with an optimal cutoff value of 128 um, and thinner nasal-quadrant pRNFLT was associated with a favorable VFI, with an optimal cutoff value of 105 um. A limitation of the study is that outcomes were not assessed beyond the 6-month time point. Clinical Ophthalmology, April 2024

Curcuminoids and Their Analogs Have Potential as Dry. Eye Disease Treatments
To evaluate whether curcuminoids and their analogs might play a role in the treatment of dry eye disease, investigators assessed the efficacy of these compounds in reducing inflammatory cytokines in cultured primary human corneal limbal epithelial cells. Results indicated that treatment with curcuminoids and their analogs significantly decreased mRNA expression levels of tumor necrosis factor-a, the interleukins 13, 6, and 17A, matrix metallopeptidase-9, and intercellular adhesion molecule-1, at rates comparable to those seen with cyclosporin A 1:500 treatment. However, cyclosporin A significantly reduced cell viability, while the curcuminoids and their analogs exhibited no effect on cell viability. BMC Complementary Medicine and Therapies, April 2024

Patients with OMG May. Also Present with Other Non-Thyroid
Autoimmune Diseases
Investigators studied the prevalence of and factors associated with non-thyroid autoimmune diseases in 208 patients with diagnosed ocular myasthenia gravis (OMG). Twenty-one patients (10.1%) were found to have other non-thyroid autoimmune diseases, with the most common being systemic lupus erythematosus and Sjogren syndrome. The only significant factor associated with the presence of these autoimmune diseases was positivity for the acetylcholine receptor antibody; therefore, screening and monitoring for other non- thyroid autoimmune diseases in this subpopulation of patients with OMG is recommended. In Asian Eyes with Lagophthalmos, a Supratarsal Implant May Provide Good Outcomes
A retrospective cohort study of 30
Thai patients with facial palsy compared the clinical outcomes
of traditional gold-weight implantation for the correction of
paralytic lagophthalmos with those of a newly designed
supratarsal gold-weight implant. It was hypothesized that the new
ellipsis-shaped implant would reduce some of the complications
seen in Asian eyes with the traditional rectangular implant,
particularly infection and exposure caused by visibility of the implant's
edge. At the 12-month follow-up visit, eyes with the
supratarsal implant had better eyelid contour and less weight migration,
weight prominence, and ptosis than the eyes with the
traditional implant. Lagophthalmos improvement was similar between

Particulate Matter Contributes to Ocular Surface Damage and
Ocular Diseases
Exposure to particulate matter (PM) presents significant health risks to the ocular surface. This review summarizes published evidence on the detrimental effects of both PM10 and PM2.5 particulates, including inflammation, oxidative stress, DNA damage, mitochondrial impairment, and inhibition of the proliferation and migration of ocular surface cells. Particulate matter can exacerbate pre-existing ocular conditions such as dry eye and allergic conjunctivitis and can contribute to new-onset development of other ocular diseases. Understanding the underlying mechanisms of PM-induced adverse effects may inform
public health initiatives.

Relay-Sutured Scleral IOL Fixation May Be an Alternative to the Yamane Technique
The authors present a novel surgical technique aimed
at optimizing sutureless scleral IOL fixation in eyes with
capsular insufficiency. Relay intrascleral 6-0 polypropylene
sutures with intraocular flanges are utilized to secure a rigid
IOL with haptic eyelets against the inner scleral wall. Three-, 6-,
and 9-month outcomes using this technique were compared
with
those following a modified Yamane approach involving flanged, double needle-guided scleral fixation with haptic trimming. Compared with the Yamane group (n = 27), the relay-sutured group (n = 26) exhibited greater mean changes in corneal astigmatism (-0.52 vs 0.44 D, respectively) and less mean IOL astigmatism (1.1 vs 0.62 D, respectively). Mean IOL decentration was similar between the groups; however, the Yamane group experienced a significant increase in IOL tilt degree and a higher incidence of persistent macular edema. Some CT Parameters Can Aid in Diagnosing Dysthyroid Optic Neuropathy
Despite the absence of definitive
diagnostic criteria and consensus around their use, computed tomography
(CT) parameters are frequently employed for diagnosing dysthyroid optic
neuropathy (DON) in adults with thyroid eye disease (TED). This systematic review
and meta-analysis aimed to assess the diagnostic performance
of orbital CT in DON. Nine articles published before July 2023
met inclusion criteria; collectively, 212 orbits with DON were analyzed. Each
article documented specific target CT parameters. The Nugent score was found to have
the highest diagnostic ability, followed by the Barrett Index
with cutoffs of 50% or 52%. The authors recommend
prioritizing these 2 CT parameters when assessing patients

Older Age and Poor Vision Appear to Lead to Worse Outcomes After Ocular Trauma
This retrospective review aimed to
identify the clinical characteristics of and associated
risk factors for poor visual outcomes following
indirect traumatic optic neuropathy (ITON), using medical chart data from
101 patients with ITON seen at 2 clinics. More than 73% of the
patients were male, and motor vehicle accidents were the most common cause of
ITONs (79%). At the final follow-up visit, 29 patients were
considered "improved" (improvement 20.3 logMAR) and 72 were
considered "unimproved" (improvement <0.3 logMAR). Multivariable analysis
identified age 240 years and poor baseline visual acuity (VA) as
significant risk factors for unimproved final VA. Intravenous glucocorticoid
administration did not appear to have a significant effect on VA improvement.

Different Types of Lacrimal Gland Tumors Have Different
Clinical Features
This retrospective case series used
data from 79 patients diagnosed with lacrimal gland lesions to investigate the clinicopathologic
features, radiologic findings, and treatment outcomes of biopsied lacrimal
gland tumors. The most common histopathologic diagnosis was immunoglobulin
G4-related disease, followed by reactive lymphoid hyperplasia
and lymphoma. Patients diagnosed with adenoid cystic carcinoma (ACC) exhibited significantly shorter symptom durations and higher incidences of proptosis compared to those with pleomorphic adenoma, and they were more prone to presenting with diplopia than patients diagnosed with lymphoma. The presence of the superior wedge sign significantly elevated the likelihood of ACC. In patients with malignant tumors, overall survival rates did not significantly differ between those with ACC and those with lymphoma. These findings emphasize the importance of urgent orbital imaging, tissue biopsy, and timely treatment initiation for patients with lacrimal gland tumors. Scientific Reports, October 2023
