Abstract A case or lethal midline granuloma in a 11-year-old Thai boy was reported. The patient developed chronic ulcer of upper and lower eyelid for four months of duration. The ulcer was not improved by antibiotics treatment. Biopsy at the rim of the ulcer revealed a necrotizing granuloma with vasculitis compatible with that seen in lethal midline granuloma. The ulcer healed within three months after steroid and radiation therapy.
