Iam
basically a Small Incision surgeon (Bluementhal)--having done over a 1000
such surgeries over the past
2 years(26yrs
in the field).
Why
would you want to make 4 stab incisions & then manuver in the AC
without anAC mainterner.Its so easy to do the Classical "Blue"
technique.where you do a straight incision-5.5mm long 1.5 mm behind
the limbus & then phalenge it out for 1 mm on either side(keeping the
Astig. "funnel' in mind. I routinely use an ACM,do a 5.5 mm
CCC(using tryptanblue),Hydro,dislocate the nucleus into the AC using
bimanual techn. & then under visco-sandwich & remove the nuclesu.If
the nucleus is large I fracture it using a vectis & Sinsky hook &
the remove it in two halves.I hardly get any post oprt.corneal edema &
the astig.is between 0.25 & 1 D.If for any reason you make an incision
more than 5.5 mm 1X suture is put.Surgical time is about 15 mts.The
greatest adv.of the BLUE.tech.is the + pressure in the eye at all times.
Just a
thought.But I guess cataract surgery is so dynamic--one can go on
improving--but what you have to keep in mind is --How easy is the technique
to teach.
